10.4 Customer Acceptance Form

____________________________, accepts the installation of the SpectraLink Wireless Telephone System as
set forth on schedule number________________ completed on ______________________, 200_.
A complete installation includes but is not limited to the following items.
1. The Master Control Unit (MCU) was installed.
2. All the Base Stations were installed.
3. Conversation was verified on all Base Stations.
4. Conversation was verified on all Wireless Telephones.
5. Hand-off was verified on all Base Stations.
6. Coverage was confirmed for the areas specified at time of facilities analysis.
7. Software was installed on customer’s PC.
8. A facilities map of the areas of coverage was given to the customer.
9. Training class was given on use of Wireless Telephones.
10. Administrative training was given to at least one user.
The following additional parts that were not part of the original order were installed:
